Malaysia currently adopts an 'opt-in' system, i.e., one must give explicit consent to be an organ donor.
Registration has to be done by completing and sending an Organ Donor Registration form. There are 3 options:
Online registration:
Register online through the MySejahtera app developed and maintained by the Malaysian government. Please click here to download the Mysejahtera app on Google Play. For instructions on how to pledge through MySejahtera, click here.
Register online through the Derma Organ website which is managed by the National Transplant Resource Centre. Please click here to pledge through Derma Organ.
Offline registration (in-person): According to the Malaysia Ministry of Health, the Organ Donor Registration form is available in all government hospitals, district health departments, district health centers. Transplant Resource Centre at Hospital Raja Permaisuri Bainon, Ipoh, Transplant Resource Centre, Hospital Penang, Transplant Resource Centre Hospital Sultanah Aminah, Johor. Offline registration (download form, print, and post): The Organ Donor Registration form can be downloaded online from the website of the Malaysian Society of Transplantation. Please click here for the link. Complete the form, sign, and post it to the address mentioned.
Speed:
The fastest option is through Mysejahtera. It would take no more than a few minutes (depending on how long you take to answer the questionnaire), and your digital organ donor card will be issued immediately.
Flexibility:
MySejahtera could be considered to be the most flexible option. After making your pledge, if you change your mind, you can withdraw the pledge immediately.
Fancy physical card:
If you'd like to have a physical Organ Donor Card, then register through Derma Organ. They will typically send the card by post. Please click here to pledge through Derma Organ.
Once registered you will receive an Organ Donor Card (digital card if you registered through Mysejahtera, physical card if you registered through other methods).
It is recommended to keep the card with you at all times.
It is recommended that you keep your family informed of your wish to be an organ donor after death.
Registration can be done online or in person.
Online registration can be done through the Sri Lanka Human Tissue Bank website. You will have the option of selecting what organs you would like to donate (such as corneas, kidneys, etc). Please click here to register online through the Sri Lanka Human Tissue Bank.

Once registered you will receive an Organ Donor certificate.
